Free Eisenhower matrix template.
Two printable sheets. A blank four-quadrant matrix for one person, and a team voting sheet for workshops. Both are free A4 PDFs. No email needed.
1. The blank Eisenhower matrix
Write each task in the box that fits. Fill in your definitions at the top first. "Urgent" and "important" mean different things to different people, so pin them down before you start.

2. The team voting sheet
Use this in a workshop when you can't use phones. Each person gets a copy and scores every item on their own, before any discussion. Score urgency and importance from 0 to 10. Collect the sheets and plot the average for each item. Then look for items where scores were far apart and discuss those first.
Team Eisenhower matrix: private vote
Score each item on your own. 0 = not at all, 10 = extremely. Don't confer until everyone has finished.

Urgency 5 or more and importance 5 or more: do it first. Importance 5 or more only: schedule it. Urgency 5 or more only: delegate it. Neither: drop it.

Skip the paper: use the online version
The paper sheet works. It's also slow. Someone has to type up every score, work out the averages and draw the chart. By the time it's done, the meeting has moved on.
The online Eisenhower matrix for teams does the same thing on phones. People vote in private, the averages and spread appear the moment you reveal, and you can export the results as a report or CSV. It's free.
Tips for filling it in
- Write definitions first. For example: urgent means it must start within four weeks. Important means it supports one of this year's three goals.
- Keep items short and specific. "Hire a paraplanner by March" sorts better than "people".
- Limit "Do it first". If more than a third of the items land there, tighten your definition of urgent.
- Name an owner for every delegated item. No name means it isn't delegated.
- Date everything in "Schedule it". Otherwise it stays there forever.

Questions
Is there an Eisenhower matrix template for Excel or Google Sheets?
Not here, on purpose. A spreadsheet is good for one person's list. For a team, the online tool collects everyone's votes and draws the chart for you, then exports a CSV you can open in Excel or Google Sheets.
Can I use these templates at work or in training?
Yes. Print and share them freely.
What size are the PDFs?
A4 landscape. They also print well on US Letter.
